Accepted methods of payment are cash, check, credit or debit cards, HSA and FSA cards.
Will insurance cover my nutrition visits?
Insurance coverage varies between insurance companies and specific insurance plans. Using the script below will assist you in verifying your nutrition insurance benefits. I am in-network with BCBS, UHC, Cigna, and Aetna.
Aetna requires a referral from your primary care doctor. Some other insurance plans may also require a referral in order to be covered by your insurance company.
Script for verifying your nutrition insurance benefits
The Katy Dietitian asks that you, the client, verify your nutrition insurance benefits.
When contacting your health insurance company to verify benefits please have your insurance card ready. Use the customer service number on the back of your card to speak with a representative from your insurance company.
Please ask the following questions to verify your insurance benefits:
- Is a doctor's referral required?
- Do I have a deductible to meet before insurance pays?
- Do I have a co-pay? If yes, what is it?
- Do I have benefits for nutritional counseling CPT codes 97802 and 97803?
- Do I have any nutrition counseling benefits covered under the preventative care portion of my plan? ( ICD-10 code Z71.3)
- Is there a limit of nutrition visits I can have?
- Are there any restricted diagnoses?
- Is telehealth covered? (if you are interested in having appointments via telehealth.
- What are my out-of-network nutrition benefits?
